Application for Police Criminal Record Check

Please note that in order to perform a police criminal record check and provide a police clearance certificate, a completed application form SF39 is required together with a fee of $100. For more information, please refer to the application form.

Please fill out the entire application form in full (answer all questions).
'Staff in Confidence' Form - if you have resided overseas, a clearance certificate must be provided for each place you have resided.
The following documents should be enclosed:
- Copy of Birth Certificate.
- Three Passport Size Photos.
- Copy of High School or College Certificate or GED.
- Three Professional Character References.
Applicants should:
- Be Bermudian;
- Or the Spouse of a Bermudian;
- Or Possess a letter from the Immigration Department stating they have permission to work in Bermuda without a work permit.
- Applicants who are children of Bermudians must have a letter from the Immigration Department stating they can work without a work permit. This is not a 'letter to seek employment.'
